We are thrilled to welcome you to our MMA Summer Camp!
Below you will find links to all of the paperwork that must be completed and/or submitted in order for your child to begin camp.
1. Emergency Information and Immunization Card
2. Religious/Personal Immunization Exemption Form
3. Topical Creams Consent Form
4. Field Trip Harmless Form
(In case of an actual “field trip”, we will have parents fill out a specific form for each
individual field trip. However, this generic form must be on file with the school for
children to leave the inner ELC gate for our nature walks and water activities, etc.)
6. Copy of Birth Certificate
(This must be on file with the school to ensure that all children are in the legal custody of
their parent or authorized guardian.)
