Below you will find links to all of the paperwork that must be completed and/or submitted in order for your child to attend their first day school.
1. Registration (If you submitted your original registration form online, there is nothing required here.)
2. MMA ELC Family Handbook Agreement
(After reading the MMA ELC Family Handbook in its entirety, only the last page must be signed and submitted.)
3. School Policies and Tuition Agreement Form

4. Emergency Information and Immunization Card
5. Religious/Personal Immunization Exemption Form
6. Topical Creams Consent Form
7. Field Trip Harmless Form
(In case of an actual “field trip”, we will have parents fill out a specific form for each individual field trip. However, this generic form must be on file with the school for children to leave the inner ELC gate for our frequent nature walks around the campus yard, etc.)
9. Private School Affidavit of Intent
(Kindergarten and Age 6+ Only. Must be notarized.)
11. Copy of Birth Certificate (a hard copy must be on file in the MMA Office)
